Registration
The registration fee for the meeting includes breakfast and breaks.
* Indiana-ACC Physician Member - $40
* Non-Member Physician - $50
* Cardiac Care Associate (RN, APN, Pharmacist, PA) or NCDR Data Manager - $30
* Practice Manager - $30
* Fellows, Residents and Students - FREE with pre-registration

Parking
The Indiana-ACC will not be validating parking. Feel free to self-park at the hotel at prevailing rates less 25%. Alternatively, you may park at The Indiana State Government Center Parking Garage adjacent to the Indianapolis Marriott Downtown. Please use the skywalk from the parking garage to the Marriott for easy access. 
 
Hotel Accommodations
Please call the Indianapolis Marriott Downtown at 877-640-7666 for room reservations in the Indiana Chapter of the American College of Cardiology room block using code "accacca." You can also make reservations online. Rooms are $152 plus tax. Reservations should be made by September 25, 2012. There is a 24-hour cancellation policy. 

Cancellations

Registration cancellation requests must be received by October 19, 2012. No refunds will be made after that date.
